Insurance Info

At 903 Physical Therapy there are a wide variety of different health plans that we accept.

We know that insurance can be confusing. As a courtesy to our patients, we will check your health benefits before you arrive to determine whether there will be a copay, deductible, or coinsurance for treatment services.

Do you have any of these insurance plans? We’ve included most of the major plans we’re in-network with; however, this isn’t an all-inclusive list! Please call the office at (903) 564-4300 to verify other insurance plans.

  • United Healthcare
  • Coventry
  • Cigna
  • Blue Cross Blue Shield ( PPO plans only)
  • Aetna
  • Humana
  • Great West
  • Golden Rule
  • First Health
  • Unicare
  • Tricare
  • PHCS
  • Medicare
  • Special Insurance Services (SIS)
  • TML-IRP

Our Whitesboro physical therapy clinic also offers a private pay plan for those patients who do not have health insurance or who have maxed out their benefits. 903 Physical Therapy offers a $150 eval then $75.00 per visit, self-paying fee.

Please contact us today for more information on our service charges and payment details.

Disclaimer: While this is an extensive list, health plans do change on a regular basis without prior notification. We recommend that you check with your health plan to clarify what physical therapy benefits are available to you.

No Surprises Act– Good Faith Estimate (GFE)

  • Any patient who is uninsured—or who is insured but does not plan to use their insurance benefits to pay for the health care services you provide—should be provided a GFE.
  • A GFE is not necessary at this time for a client or patient who is planning to use their insurance benefits to cover services you deliver.

The GFE provision does not apply if the client is a participant in Medicare, Medicaid, or other federal healthcare programs. GFE is also not generally required for emergency services or services that cannot be scheduled in advance.

Testimonials

Corey Hartless

Outstanding staff! Have been extremely helpful in progress of shoulder after rotator cuff surgery. They truly care about their patients and their recovery journey!

Radena Strong

They are professional but at the same time make you feel very welcome. They have helped me gain strength and I'm confident in my recovery. They have former patients that come to work out and socialize. That says a lot that they want to continue even after they are recovered.

Joshua A.

I started therapy for rotator cuff pain. Limited motion and back pain. I have improved tremendously. My back is relatively pain free and significant improvement in my range of motion and less pain.
